Why the AKU admission test?

Once you've submitted the application form, you should, within a few days, receive a mail from AKU containing your admit card for the test. The AKU Application Form - Tips and words of Caution

The AKU application forms come out in Januaray evry year and cost about Rs.200 each. As far as i remember, you get the forms from AKU directly. Around January every year, AKU starts giving out its application forms. Once they do that, they will host an 'Open Day' at their super-awesome Gymnasium. I would HIGHLY recommend attending it as it's quite informative. They'll tell you lots of stuff about AKU and will also explain the format of the test themselves.
